Farmers Insurance Group® - Consumer Interior Survey
In order to complete the survey process, it will be necessary for one of our
professional Field Reporters to call and set up an appointment date to conduct
their onsite visit.
Before completing the online order form, the following information
is needed:

The Insured's name and address...

The Insured's telephone number (home, work and cellular number if
applicable)...

The best time to call to set up an appointment (no weekend appointments)...

Credit card (Visa or MasterCard)...
Please note the following:

Payment for services must be made by credit card at the time of order placement.

The charge for services is based upon
the geographic location of the property.

A $25.00 non-refundable processing fee will be charged to your selected credit card at the time of sale.

The remaining balance will be charged to your credit card upon completion of your final report.

The Field Reporter will make a maximum of (3) three attempts over a 7-10 day period to contact you
via telephone to schedule the onsite visit. If a return response is not received by you at the conclusion
of this period, or you fail to make a scheduled appointment, the survey request will be closed-out and
the $25 processing fee retained by AFS. No further contact will be attempted by the Field Reporter.
If at any point in the future you should decide to proceed, a new survey request must be submitted
by you. This new request will also require a $25 non-refundable processing fee. There are no exceptions
to this policy.

Multi-unit properties will be assessed an additional charge of $35 per unit (from units 2 to 4).
A Field Reporter will be assigned to your request within (2) business days. Within (5) to (7) business days
after assignment, you can expect a call from this individual to setup an appointment. The survey will then
be completed, reviewed, and an e-mail will be sent to you with instructions on how to retrieve your report
online. This e-mail will be sent to the address you specify (7) to (10) days after the date of appointment.
If upon final review of your survey you decide to participate in the Farmers Consumer Interior Program,
you must contact your Agent directly. Your Agent will be able to provide you with specific instructions on
how to proceed and where to deliver your completed survey to in order to begin the process.
The entire process should take no more than (21) days with your cooperation.
